Job Title: SERVICE COORDINATOR II, Enhanced Service Coordination – Bilingual Required

 Job Summary: Within a team environment, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance. The position will collaborate with the Person-Centered Practices Specialist to ensure knowledgeable and use of person-centered planning, and how it applies to supporting this specific group of people and families. This position will collaborate with the Benefits Specialist and the Multi-Cultural Specialist to ensure they are using innovative approaches to service coordination and accessing generic resources.

Specific Job Duties:

  1. Manage a 1:40 caseload ratio currently served by TCRC with the low POS and/or no POS
  2. Utilizing a Person-Centered approach to the Individual Program Plan Process, will work to identify barriers is accessing services and supports; to enhance relationship between individual/family and TCRC; to empower individuals and families to partner with TCRC and other community agencies and to assist in identification of new resources that are culturally relevant to their community.
  3. Works in collaboration with the PCP Specialist, Cultural Specialist, Self-Directed Services team and other TCRC Departments to identify innovative approaches to service coordination.
  4. Using PCP strategies will be responsible for the development and implementation of the individual’s IPP, progress reports, annual reviews, and quarterly in person reviews.
  5. Provides follow-up, reassessments, and periodic review of individual’s progress and program
  6. Adheres to the agency mission, philosophy and core
  7. Assists in locating and developing natural
  8. Initiates the purchase of vendored and generic services and closely monitors service delivery. As well as assists in navigating state-wide systems such as Departments of Managed Health Care, Social Services and Rehabilitation.
  9. Keeps abreast of state and federal regulations, agency policy and procedure and guidelines related to services for individuals. Acts as an advocate on social, legal, education, mental health and/or forensics issues related to the individual.
  10. Participates in outreach and represents the agency in community meetings and
  11. May conduct public presentations regarding regional center services and
  12. May serve on intra-agency and/or inter-agency
  13. May participate in data gathering and analysis
  14. Additional duties may be assigned as deemed appropriate\(i.e. community presentations, attending local resource fairs, etc.)

Primary Job Functions:

  1. Conduct strength-based assessments that communicate an understanding of the unique issues facing family members.
  2. Involve individuals we serve and family in all aspects of person-centered planning and support
  3. Design, document, implement and monitor services and supports in a timely
  4. Routinely contact individuals and families to answer emerging questions, quarterly in-person review meetings and annual IPP or IFSP meetings.
  5. Facilitate planning team progress, identifying outcomes, needed services and Ongoing data collection.
  6. Develop and maintain a network of vendors and community
  7. Provide relevant information and training about developmental disabilities, services and
  8. Use effective communication and interpersonal skills that consistently respect children and adults with developmental disabilities and their families of varying cultural backgrounds.
  9. Demonstrate an operative knowledge of family/natural support and private/public resources and integrate these into service planning resulting in community-based outcomes.

Minimum Qualifications: 

  • Master’s or Bachelor’s degree in human services field or a related area, with 2 years of applicable experience in mental health or social work.
  • 2 years experience case management within a related
  • 1 year experience with individuals with developmental
  • Experience in using Person Centered Thinking (PCT) and Person-Centered Planning (PCP)
  • Bilingual and bicultural English/Spanish –
  • Professional and lived experience in working with families from diverse cultural, linguistic, and socio-economic backgrounds.

Physical Requirements

Adequate manual dexterity and coordination for operation of a computer and other standard office equipment is required for this position. The ability to sit at a workstation for long periods of time, the ability to read, write and comprehend large amounts of written material such as reports and regulations, use the telephone, and communicate via electronic mail are all required on a daily basis. The ability to travel to another work-site location is absolutely required on a daily basis. The ability to stand, sit or walk for moderate distances on uneven terrain is also required on a daily basis. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, feel or operate objects, tools, or control and reach with hands and arms. The employee frequently is required to stand, sit, walk, push, pull, kneel, stoop, bend, and squat. The employee must frequently lift and/or move up to 10 lbs and infrequently up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus. Each of these physical tasks is an essential function of the position.
