Job Title: SIR Coordinator
Job Summary: Review and edit in a person-centered manner all Special Incident Reports reported to TCRC for completeness, accuracy and consistency, in accordance with Title 17 requirements. The SIR Coordinator is responsible for timely submission of all Special Incident Report documentation to DDS. Special incident follow-up will include findings and corrective actions to assure the health and safety of those served by TCRC to prevent the recurrence of system problems. The SIR Coordinator also facilitates the Risk Management Committee, provides training to staff and is a point of contact for DDS Liaison in the area of special incident reporting. Additionally, the SIR Coordinator provides the TCRC delegated conservatorship responsibilities for individuals for whom DDS is the appointed conservator. Additional duties may be assigned when necessary.
Job Duties & Responsibilities:
Primary Functions:
SIR Coordination:
1. Coordinates and assures timely implementation of TCRC’s overall SIR process including reporting to DDS and follow up process.
2. Reviews all TCRC SIR’s, interacts proactively with Service Coordinators / Services and Support Managers, investigative agencies and local offices as appropriate.
3. Provides information to the Quality Assurance Manager regarding possible need to monitor providers to ensure compliance with SIR related corrective action plans.
4. Works with Quality Assurance staff and the vendor community addressing SIR trends.
5. Upon receipt of After-Hours Communication Logs identifies SIR’s, communicates to Service Coordinator, Services & Supports Manager.
6. Identifies needs and provides training to Services and Supports staff regarding Special Incident Report processes and other risk mitigation processes.
7. Review and process Department of Developmental Services (DDS) MediCal claims in relation to Special Incident Reports.
Risk Management:
1. Meets quarterly with the Risk Management Committee. Identifies and recommends training topics. Identifies trends and patterns in SIRs, communicates findings to Assistant Director and Manager of Federal Programs and the Risk Management Committee.
2. Tracks statistics regarding SIRs as requested by Director of Services and Supports or the Risk Management Committee.
3. Will monitor the occurrence of adverse events, captured through Special Incident Reports (SIR), to identify trends and develop strategies to prevent and mitigate risks to ensure the health and safety of those we serve.
4. Participates in the Department of Developmental Services (DDS) Statewide Risk Management Committee.
5. Acts as a liaison with the Department of Developmental Services (DDS) in relation to Special Incident Reports and Risk Management trends.
DDS Delegated Conservatorship Responsibilities
1. Monitor the conserved individual’s health, safety and well-being, and protection of their rights: including monthly updates to DDS of any changes.
2. Meet at a minimum quarterly with the individual who is conserved by the Department of Developmental Services (DDS)
3. Maximize the conserved individual’s autonomy and support the individual in making their own decisions to the greatest extent possible.
4. Provide support to the individual conserved at IPP and planning team meetings to ensure there are supports in place that allow for the individual in becoming more independent, increasing their decision-making abilities, and exploration of alternatives to conservatorship.
5. Provides a comprehensive person-centered biennial assessment of the individual conserved and their needs.

To Perform effectively in this position the individual must possess the following:
1. Demonstrated knowledge and skills in the field of Developmental Disabilities, quality assurance practices, service coordination, and current regulatory information.
2. Demonstrated strong organizational skills and the ability to manage and monitor multiple tasks. Ability to problem solve in a flexible and effective, independent manner in carrying out job responsibilities.
3. Outstanding interpersonal skills in communicating effectively with diverse peoples in a variety of situations and on a wide variety of issues.
4. Ability to work collaboratively across the organization within a team service model.
5. Excellent writing skills particularly in report writing.
6. Math skills necessary for analysis.
7. Exhibits a strong attention to detail.
Physical Requirements:
Adequate manual dexterity and coordination for operation of a computer and other standard office equipment is required for this position. The ability to sit at a workstation for long periods of time, the ability to read, write and comprehend large amounts of written material such as reports and regulations, use the telephone, and communicate via electronic mail are all required on a daily basis. The ability to travel to another work-site location is occasionally required. The ability to stand, sit or walk for moderate distances is also required on a daily basis. While performing the duties of this job, the employee is regularly required to use hands and fingers to handle, feel and operate objects, tools, or control and reach with hands and arms. The employee frequently is required to stand, sit, walk, push, pull, kneel, stoop, bend, and squat. The employee must frequently lift and/or move up to 10 lbs. and infrequently up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus. Each of these physical tasks is an essential function of the position.
