Job Summary: Serves a member of the Community Placement Program (CPP) team in the Community Development Department (CDD), providing Nursing assessment as one component of the community transition plan for people moving from State Developmental Centers or other restrictive settings to the community. Integrates nursing assessment and recommendations into a whole person transition planning assessment process. As directed by the CPP manager, provides person centered nursing consultative services to Tri-Counties Regional Center (TCRC) staff for individuals currently residing in the community, especially those who have recently moved from State Developmental Centers or other restrictive settings
Primary Functions:
- Serve as a consultant to professional staff in assessing the health management needs of individuals with developmental disabilities residing in the community or Developmental Center or other restrictive settings.
- Participates on the Individual Planning Team for individuals in the Developmental Center or other restrictive settings that are under consideration for community placement, especially where health care planning is involved.
- Develop through systematic assessment appropriate nursing care plans, while advocating for needed or anticipated services for individuals TCRC serves in the least restrictive environment.
- Provides consultation and support for individuals residing in the Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN) homes as required by Title 17 and Lanterman Act.
- Evaluate current nursing care plans for persons residing in the community and develop recommendations for improved health care.
- Locate nursing care services in the community to provide varying levels of care to individuals TCRC serves in their homes.
- Assist in developing needed community services related to the health and wellbeing of individuals TCRC serves.
- Assist service providers to develop and health care and maintenance plans as they identify medical needs and mediation issues.
- Consult on medical issues/needs and coordinate with current services and programs that would serve individuals.
- Assist in planning and implementing community educational programs related to health and wellbeing of individuals.
- Provide consultation to other professionals, agencies, and to providers of care as coordinated through the CDD/CPP programs.
- Complete nursing reports.
- Work with Quality Assurance personnel on monitoring the implementation of health care plans or practices, staff training issues, and development of corrective action plans for service providers on health and health related issues.

- able to spend 30% – 50% of work time traveling throughout TCRC area
- valid California driver’s license and reliable, insured vehicle or equivalent means of transportation
Education and Experience:
Current license as a Registered Nurse in California, or Master’s Degree in Nursing. A minimum of 4 years of professional nursing experience with 2 years of experience in a community based health agency. Experience working with individuals with developmental disabilities and mental illness desired.
