09102-Developing the Standardized Individual Program Plan
SCOPE:
This procedure applies to all Service Coordinators and Services and Supports Managers of Tri-Counties Regional Center.
PROCEDURE:
- Every individual must have a Standardized Individual Program Plan (S-IPP) that reflects the desired outcomes of the person served. The plan should identify both the generic and paid supports that reflect choice, program and cost effectiveness.
- The Standardized Individual Program Plan meeting must be reviewed face-to-face with the individual and/or their legal representative if the individual has not been seen in person within the past 12 months. Otherwise, the S-IPP meeting may be held remotely only when requested by the individual or family (Reference: DDS Directive G-2025 Case Management – 001)
- Individuals and families are encouraged to invite other significant people to the planning meeting to provide information in developing a plan which identifies and supports the desired outcomes.
- The S-IPP meeting must be held at a time and location convenient for the person.
- The Standardized Individual Program Plan document must:
- Specify the approximate scheduled start date for services and supports, and outline timelines for actions necessary to begin services provided by TCRC and/or generic services.
- Include schedule of the type and number of services and supports to be purchased by the regional center or obtained by the generic agencies or other resources.
- Identify the provider/s responsible for achieving each objective (i.e. vendors, generic service agencies and natural supports).
- Be signed by the individual or authorized representative prior to implementation of services and supports.
- Individual served, the family or their legal representatives may request a Standardized Individual Program Plan (S-IPP) meeting at any time.
- The S-IPP meeting must be held within thirty (30) calendar days when creating new S-IPP or an amendment to the existing S-IPP (Reference: Welfare and Institution Code Section 4646.5 (8)(b)
- If there is no agreement to one or more elements of the plan, a follow-up meeting must be held within (15) calendar days or later at the request of the individual, the family and/or their legal representative (Reference: Welfare and Institution Code Section 4646 (h))
- New intakes and transferred individuals to Tri-Counties Regional Center from another Regional Center must have a Standardized Individual Program Plan.
- Individuals new to TCRC; a S-IPP must be developed within sixty (60) calendars days of their eligibility determination. (Reference: Welfare and Institutions Code Section 4646 (c)).
- Individuals transferred to TCRC from another Regional Center area must have a S-IPP developed within thirty (30) calendar days of the acceptance date of transfer. (Reference: Welfare and Institution Code Section 4643.5 (c).
- Individuals transitioning to the community from a state development center will have an S-IPP developed within (30) calendar days of their placement and will receive enhanced case management services. The Service Coordinator will:
- Be present during the pre-placement visit and on the day of placement to support a smooth transition.
- Ensure that the community provider is prepared to meet the individual’s health and safety needs by implementing the transition plan as outlined by the developmental center’s discharge plan.
- Meet face-to-face with the individual in the community at a minimum of once every (30) days for the first (90) days of community placement.
