09204-Responding to the Request for a Change in Service Coordinator
SCOPE:
This procedure applies in any instance when a person, a person’s family member or any other legal representative on behalf of the individual, requests a change in the Tri-Counties Regional Center service coordinator assignment.
PROCEDURE:
- A request for a change in the service coordinator assignment received from the person receiving TCRC services, the person’s family member or any other legal representative on behalf of the person, signals a team and manager to focus on an examination of the problem issues as perceived by:
- The person receiving TCRC services;
- The family or other legally authorized representative; and,
- The service coordinator.
- Any change in the service coordinator assignment may cause disruption to the continuity of service coordination and is to be avoided to the extent possible.
- The team or manager provides supportive problem solving for the service coordinator and the person served by TCRC, family, and representatives regarding the identified problem issues in order to mediate the situation with the current service coordinator.
- The team and/or manager takes this opportunity to assist each other in assessing all issues with the person served by TCRC, family members and/or other parties.
- This supportive mediation may allow all concerned parties to understand the issues from each other’s perspectives, and to re-engage in a more collaborative relationship.
- If after attempts to resolve the concerns among the parties, the individual and/or family continue to want a change in the service coordinator assignment, the manager facilitates this change:
- The manager facilitates the service coordinator assignment that is acceptable to all parties.
- The current service coordinator co-manages the case with the newly assigned service coordinator until the newly assigned service coordinator is familiar with the person served, the family and the current situation. Depending on the complexity of the situation, this transition may take one (1) or two (2) months.
- All Tri-Counties Regional Center service coordinators follow Tri-Counties Regional Center service policies. Therefore, a change in the service coordinator assignment does not change policy-driven service coordination decisions.
