Coordination to the Transition to Adult Services

SCOPE:

This procedure applies to all Services and Supports Department employees of Tri-Counties Regional Center. Tri-Counties Regional Center service coordination staff will participate in transition planning activities for secondary students over the age of 16. Participation by service coordination staff will be based upon consent of the family for students ages 16 through 17, and of the student ages 18 and up, or upon consent from the legally authorized representative.

PROCEDURE:

  1. Tri-Counties Regional Center:
    1. Allocates staff resources to participate in individual transition planning meetings;
    2. Develops transition liaisons to work on local Interagency Transition Team(s) in order to coordinate local resources and develop resources where needed;
    3. Participates in the development of Memoranda of Understanding among transition partners to ensure:
      1. 1. All appropriate agencies are participating in transition planning;
      2. 2. Transition planning fulfills the legal mandates;
      3. 3. Transition planning assists students to reach his/her long-range life goals;
      4. 4. Transition planning develops activities and needed accommodations that are included in the Individual Transition Plan to assure the student’s school program and related services will prepare him/her for the adult world;
      5. 5. Appropriate resource development activities are occurring to develop services determined necessary but not yet available; and
      6. 6. A means for interagency cross-training to improve coordination or services and address barriers to implementation of transition plans.
    4. Evaluates all students for potential regional center eligibility if previously unknown to regional center.
  2. Tri-Counties Regional Center service coordination staff:
    1. Upon invitation by the legally authorized individual, participate in the development of transition plans and annual monitoring until such time as a successful transition to adult services has been completed;
    2. Utilize a person-centered planning approach to transition planning. Emphasis will be placed upon the individual student’s interests and preferences as well as strengths, hopes, and dreams for the future, both by the student and family;
    3. Demonstrate high expectations for meeting the individual student’s hopes and dreams;
    4. Participate in a cooperative and collaborative manner within the transition planning team as an equal interagency partner;
    5. Provide opportunities for education of all members of the transition planning team;
    6. Respect the choices of the student and family;
    7. Communicate actively in the development of the individual transition plan;
    8. Clarify issues such as eligibility, assessment or financial requirements for necessary services;
    9. Ensure that appropriate consideration has been given to all appropriate alternatives;
    10. Ensure consideration has been given to transportation necessary for successful transition services;
    11. Provide access to information about resources, requirements and services as requested;
    12. Document transition planning activities in Sandis Title 19 Notes and appropriate amendments to the regional center’s IPP; and,
    13. Coordinate funding and supports necessary to ensure a successful transition plan is implemented.
  3. Reassignment of case management responsibility from the Transition Service Coordinator to the Adult Team Service Coordinator. 
    1. Transfer of case management responsibility will occur after the sending and receiving service coordinators have discussed the needs of the individual and an appropriate transfer date has been identified.   The receiving service coordinator will be introduced to the person prior to the transfer, whenever possible. 
    2. Generally, the Transition Service Coordinator will be responsible for managing the transition to adult programming.  However, to more equitably manage the workload, an office may decide to have the Adult Team manage that function.  The service coordinator responsible for managing the transition will complete all related documentation, including referrals to DR/Habilitation, amendment of the IPP to reflect adult programming and purchase of services, and arrangement of transportation.  
    3. Timing of the transfer may vary depending upon the needs and preferences of the individual.
