12102 Reviewing the Annual, Quarterly and or Tri-Annual Standardized Individual Program Plan

SCOPE:
This procedure applies to all Service Coordinators and Services & Supports Department Managers of Tri-Counties Regional Center.
 
 PROCEDURE:
 
Each person served by Tri-Counties Regional Center (TCRC) must have their progress monitored through the regular annual, quarterly and/or /tri-annual review of their Standardized Individual Program Plan (S-IPP).
These   reviews are conducted by the service coordinator with the individual served or family. Other people being involved is encouraged and may be invited to participate as desired and approved by the individual served.
The service coordinator must conduct an in-person S-IPP if the individual has not been seen in person in the past 12 months. Otherwise, the S-IPP meeting may be held remotely only when requested by the individual served or family. (Reference: DDS Directive G-2025 Case Management – 001)
The service coordinator completes the review and amends the S-IPP as needed.
The service coordinator is required to conduct an annual review when the individual resides in the family home. Additionally, the service coordinator must conduct in-person quarterly monitoring reviews when:
The adult is living outside the family home; this includes individuals who may be unhoused or incarcerated.
The child is aCounty Dependent Youth residing in a Resource Family Home. This placement may be with non-relatives or with relatives and is supported through county involvement.
 
For individuals living with family, and/or conservator who are not on the Federal Home and Community-Based Services (HCBS) Waiver:
Their Standardized Individual Program Plan is updated at least every three years, with annual contacts as requested by the individual and/or family.
For individuals served in the family home who are on the Federal Home and Community-Based Services (HCBS) Waiver:
Their S-IPP is reviewed annually by the service coordinator, the individual, and their planning team using the Standardized Annual Review Form. (DDS 2201) to ensure all goals and services are up to date.
 
For individuals living in the regional center or Medi-Cal funded residences (i.e. Community Care Facilities, Intermediate Care Facilities, Supported Living arrangements):
Their S-IPP is reviewed quarterly, with one of these reviews serving as the Annual Review. During this visit, the plan is evaluated and updated as needed to reflect any necessary adjustments or amendments.
