Salary: $31.90 – $35.16 hourly (full range $31.90 – $39.89 hourly)

Job Summary: Serve as the Agency’s Service Coordinator who serves individuals newly placed in the community from Developmental Centers (first 12 months in the community, or until stable). Provides consultation, advocacy, resource information and service coordination to these individuals. Anticipated caseload for this position should be 20 to 35 cases, although the state’s limit is 45 cases. Considerable driving required. Reports to the Community Placement Plan Manager.

Primary Functions:

  1. Conducts strength-based assessments which communicate an understanding of the unique issues facing family members.
  2. Involve consumer and family in all aspects of person-centered planning and support
  3. Design, document, implement and monitor services and supports in a timely
  4. Facilitate planning team progress, identifying outcomes, needed services and
  5. Develop and maintain a network of vendors and community
  6. Provides relevant information and training about developmental disabilities, services and resources.
  7. Use effective communication and interpersonal skills, which consistently respect children and adults with developmental disabilities and their families.
  8. Participation on IDT (interdisciplinary team), required by 1039 & 391, “Service Coordinators must be part of IDT”, including participation with IPP planning and transition planning.
  9. All Special Incident Reports must be called into the Developmental Center Staff, these verbal reports of incidents should be recorded in the T19 notes, as well as written notices filed in the individual’s SIR’s involving a death must be reported to DDS as well.
  10. To advocate for individuals to reside in the least restrictive environment per regulations in the Lanterman Act, to assist with identifying needed services to continue to reside in the least restrictive environment, and advocate for provision of such services, even if this may be against conservators wishes, per the Lanterman Act.
  11. Effective verbal or written communication with individual, staff and family on a minimum quarterly basis or as requested.
  12. Documentation of all interactions with individuals, family, staff and those involved with the individual in T19 notes.
  13. To monitor court re-commitment dates, including 6500 and In re Hop (court commitments) and to follow up with attorney to ensure timely compliance with these and other legal matters.
  14. To direct to the individual’s chart any information or pertinent legal documents on a individual following review of information

Skill Requirements:

  1. Knowledge of developmental disabilities
  2. Awareness of entitlement and benefit programs
  3. Ability to access service delivery systems and operations
  4. Awareness of public benefit programs
  5. Problem-solving ability
  6. Verbal and written presentations to groups
  7. Ability to work independent

Minimum Qualifications:

  1. Master’s degree in human services field or a related area, or bachelor’s degree with 3 years of applicable experience.
  2. Familiar with Community and Supported Living facilities
  3. Familiar with Dual-Diagnosis and/or Mental Health issues
  4. Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation
  5. Able to spend 30% – 50% of work time Overnight stays infrequent.
  6. Desire strong documentation skills, including knowledge of Medical Waivers
  7. Able to read/speak/write Spanish and English preferred

Physical Requirements:

Position requires the ability to work independently. Must possess mobility to work in a standard office setting and use standard office equipment, including a computer. This is primarily a sedentary office classification. The position also requires grasping, repetitive hand movement, finger dexterity and fine coordination in preparing statistical reports and data using a computer keyboard and calculator. Additionally, the position requires near, far, and color vision in reading correspondence, statistical data, and using the computer. Hearing and speech are required to communicate in person, before groups and over the telephone.
Employees must possess the ability to lift, carry, push and pull materials and objects necessary to perform job functions. The employee must frequently lift and/or move up to 10 lbs. and infrequently up to 20 lbs. The position requires standing, walking, reaching, twisting, turning, kneeling, bending, squatting, and stooping in the performance of daily office activities. Work is performed in an office setting with moderate noise levels, controlled temperature conditions and no direct exposure to hazardous physical substances. Employees may encounter upset individuals over the phone and/or in person. Each of these physical tasks is an essential function of the position.
